Food is Medicine Toolkit for Type 2 Diabetes

Overview
The Food is Medicine Institute will leverage the expertise and resources within Kaiser Permanente and the broader field to address current gaps and barriers in Food is Medicine (FIM) implementation through the development of pragmatic resources. Activities include evidence review, development, curation and dissemination of FIM clinical education, and culturally /linguistically relevant patient material, in addition to facilitated engagement and partnership development with community providers. It will also partner with healthcare plans and health systems in the development of a FIM business value framework. The insights, inputs and learnings from healthcare partners will inform the development of the FIM Playbook, a practical and accessible resource for FIM practitioners across the nation.
Project Aims
Aim One
Provide FIM subject matter expertise to support the innovation of healthcare stakeholders.
Aim Two
Partner with health plans and health systems to develop a business value framework for FIM by line of business.
Aim Three
Aim to increase the quality of the FIM interventions by developing the FIM Toolkit and disseminating it via public website.
Aim Four
Provide group and peer learning sessions for FIM providers and partnership development online resources to support community health centers.
Aim Five
Develop culturally and linguistically appropriate FIM patient education resources.
Aim Six
Develop and provide a series of clinical education opportunities using “train the trainer” models to increase clinician and staff knowledge and skills.
Aim Seven
Deliver a FIM “Going Deeper” Implementation Guide.
